Job Title: Clinician, Housing Support Services Program (HSSP)

Dept: Mental Health Services

Schedule: Monday-Friday, 8:00am-4:30pm

Location: Long Beach, CA

SUMMARY

The Housing Supportive Services Program (HSSP) is a specialized program that provides comprehensive and intensive field-based community mental health services designed to meet the unique needs of individuals in Permanent Supportive Housing (PSH) who are formerly homeless and have a Severe Mental Illness (SMI) or Serious Emotional Disturbance (SED). As part of a multi-disciplinary team, the Clinician will conduct psychotherapy with adult members that have severe and persistent mental illness in the office, in member's homes, or the field. The Clinician's primary responsibilities are to engage members and to coordinate mental health and recovery services that will ensure housing stability and community integration. This position is responsible for maintaining the highest quality provision of direct individual and group therapeutic services and case management services, while fostering teamwork among staff and across departments and programs. The Clinician's primary work site will be the assigned Project Based Housing site, Bloom: 1770 Magnolia Ave, Long Beach, CA 90806

ESSENTIAL DUTIES & RESPONSIBILITIES



  1. Provide, coordinate, and implement mental health and recovery support services with individual members
  2. Conduct individual, family and group psychotherapy
  3. Develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health
  4. Collaborate with each member to develop their individual service plan; review and update it upon completion of goals
  5. Assist management and other Clinicians with program evaluation and modification for future program improvements and growth
  6. Assist members with becoming involved in daily activities, socialization, health maintenance, sense of community, and progress in recovery reflective of The People Concern's model which emphasizes customer choice, harm reduction, and psycho-social rehabilitation
  7. Work cooperatively and cohesively with other members of the HSSP team, including participation in weekly staff meetings, group clinical supervision, and trainings
  8. Develop and maintain a complete, accurate, and current member file with all funder-required documents


Qualifications




  1. Master's degree in Social Work, Psychology, or a related field of study from an accredited university.
  2. Valid registration or licensure with the California Board of Behavioral Sciences.
  3. Current, valid California Driver's with an acceptable driving record and reliable vehicle
  4. Able to transport members in your vehicle
  5. Outstanding clinical and strong team-building skills
  6. Detail-oriented with excellent time management, organizational, written, verbal, and computer skills



Work Environment

* Combination of either field and/or office environment depending on position

* On occasion walk or drive to different agency sites

* May/Will necessitate working in busy and loud environments depending on position

* May/Will be exposed to elements like cold, heat, dust, noise and odor depending on position

* May need to bend, stoop, twist and sit throughout the day

* Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
