Short Description

The Mobile Integrated Health Care (MIH) Provider will enhance the healthcare continuum through targeted inhome care and care coordination. The MIH model expands the traditional role of EMS by shifting from a strictly emergencyresponse framework to a proactive, preventive, and patientcentered approach. The MIH provider operates as a mobile clinical resource that bridges gaps between EMS, primary care, hospitals, and social service agencies-ultimately improving outcomes for highrisk and vulnerable populations, decreasing ED visits and readmissions and decreasing over-utilization of the 911 system.

Responsibilities

  • Conduct Comprehensive In Home Assessments. Identify medical, behavioral, and social needs in the patient's home environment.
  • Communicate and Coordinate with Healthcare Providers. Relay findings to primary care, care managers, and specialty teams to help close care gaps.
  • Coordinates access to community-based health services in underserved and general populations, including primary care, public health, community-based programs, disease management, prevention and wellness, and mental health.
  • Identifies changes in patient status that require immediate initiation of 911 emergency services and/or communication with the appropriate Transitional Care member and/or other medical providers managing the patients care to determine treatment plans.
  • Provides clinical care, up to NJ paramedic scope of practice, in accordance with NJAC 8:41 & 8:49 regulations and agency specific clinical protocols
  • Serves, as part of a multi-disciplinary team, as a natural bridge to support persons with SUD and their families in accessing care or support
  • Provides follow-up, in partnership with a peer recovery coach/patient navigator, and as part of a multi-disciplinary team, to patients recently reversed with Naloxone and/or administered opioid partial agonist-antagonist
  • Supports Chronic Disease Management. Monitors and reinforces care plans for chronic conditions such as CHF, COPD, diabetes, and hypertension and SUD.
  • Provides post-discharge home visit, with the goal of reducing morbidity and hospital admission/ readmission.
  • Provides education to first responders regarding resources available to SUD patients to better prepare them to provide this information to patients/families.

Experience Required

3-5 years of experience required, in advanced life support (ALS) environment as a paramedic

Skills/Qualifications:

Communication-Ability to communicate with patients, visitors and coworkers required

Bi-lingual English/Spanish skills preferred


Education Requirements

High School/GED required

Bachelor degree required, in Healthcare or Public Health related field

Degree requirement waived with paramedic experience of 10 years or more


License/Certification Requirements

NJ Mobile Intensive Care Paramedic (MICP) or Mobile Intensive Care Nurse (MICN) required, or eligible for NJ reciprocity.

NREMT-Paramedic preferred.

Community Paramedicine Certification (CP-C) required or obtain within 1 year of hire

BLS, ACLS, PALS, EPC, or PEPP upon hire
