Job Summary

The Patient Navigator 2 provides non-clinical navigation and coordination for the Alzheimer Therapeutics Program (ATP) within the Neurology Clinic, supporting patients being evaluated for or receiving disease-modifying anti-amyloid therapy. Serving as the primary non-clinical point of contact for patients and care partners, the Navigator guides them through a complex treatment pathway that may include eligibility evaluation and biomarker confirmation, treatment by infusion or injection, required MRI surveillance, and ongoing monitoring.

The Patient Navigator coordinates scheduling, authorizations, medical record retrieval, patient materials, tracking, and communication across Neurology, Neuroradiology, Infusion Services, and Specialty Pharmacy, helping ensure patients progress through required treatment milestones. The position identifies non-clinical needs and barriers to care and routes clinical questions, symptoms, and treatment concerns to the appropriate registered nurse, advanced practice provider, or physician in accordance with program protocols.

The Patient Navigator also coordinates and participates in case conferences, administers cognitive testing following required training and certification, and assists with patient submissions to the Alzheimer Disease database. Through consistent communication, coordination, and follow-through, the Navigator plays an important role in helping patients and care partners successfully navigate this specialized treatment program.

  • A Bachelor's degree in a related area or equivalent experience/training
  • Experience in case management
  • Experience in referral coordination
  • Acquiring knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations
  • Knowledge of Medical Terminology
  • Knowledge of data collection, compilation, and analytical techniques
  • Skills to comprehend and assess patients' grievances to quickly locate appropriate resources for assistance
  • Working knowledge of the organization and how to get issues resolved
  • Solid interpersonal and customer service skills
  • Ability to be diplomatic and highly organized
  • Ability to remain calm under pressure and apply sound judgment
  • Ability to work independently in a fast-paced, demanding environment with minimal supervision
  • Ability to manage and oversee multiple tasks simultaneously, including high daily call volume
  • Ability to communicate effectively, both orally and in writing
  • Proficiency with Windows-based software including Microsoft Word, Excel, Outlook. Knowledge of computer systems and software used in the functional area
  • Ability to communicate effectively and patiently with individuals with memory impairment and their care partners, including use of repetition, written reinforcement, and care partner involvement
  • Working knowledge of Epic or a comparable electronic health record, including referral workqueues, scheduling, and in-basket workflows
  • Ability to follow defined program protocols precisely, including distinguishing non-clinical questions from clinical concerns requiring hand-off to licensed staff

Department Overview

The Department of Neurology advances the understanding and treatment of neurological disorders through specialized patient care, research, and multidisciplinary collaboration. The Department provides care for adults and children with a broad range of neurological conditions, including Alzheimer's disease and other dementias, epilepsy, movement disorders, multiple sclerosis, neuromuscular conditions, headache, and sleep disorders.

Within the Section of Cognitive and Behavioral Neurology, the Alzheimer Therapeutics Program (ATP) provides FDA-approved disease-modifying treatments for patients with early symptomatic Alzheimer disease. The program uses a structured, safety-focused approach to coordinate eligibility evaluation, treatment, monitoring, and longitudinal follow-up.

ATP's multidisciplinary team includes cognitive neurologists, an advanced practice provider, a registered nurse, a Patient Navigator, a Patient Services Representative, and a Medical Assistant, with support from Neuropsychology and Social Work. The team partners closely with Neuroradiology, Infusion Services, and Specialty Pharmacy to provide coordinated, patient-centered care throughout the treatment journey.
