Position Summary:

We have an exciting opportunity for a Certified Wound Ostomy Nurse (CWON) to join our team in Duarte, CA. This role involves collaborating with interdisciplinary teams to provide expert care, education, and support to patients and their families. The CWON will manage the prevention, treatment, and healing of chronic and acute wounds, provide ostomy care, and offer solutions for incontinence-related issues. Your adaptability and commitment to delivering exceptional care will make a significant impact on our patients' cancer journey. This position is listed for full-time, 10-hour shifts (4 shifts per week), day shifts, and will be required to work both weekdays and weekends.

As a successful candidate, you will:

• Perform comprehensive assessments of wounds, ostomies, and fistulas.
• Develop, implement, and evaluate individualized care plans for patients with ostomies or complex wounds, ensuring evidence-based practices.
• Collaborate with oncologists, surgeons, nurses, dietitians, and other healthcare professionals to ensure integrated, holistic patient care.
• Provide patient and caregiver education on ostomy management, skin care, wound prevention, and product use.
• Assist with ostomy appliance fitting and troubleshooting to prevent complications and enhance patient comfort.
• Monitor wound healing and adjust treatment protocols in coordination with the interdisciplinary team.
• Document all patient interactions and care plans accurately in the electronic medical record (EMR).
• Participate in quality improvement initiatives and wound care audits.
• Serve as a clinical resource and mentor for staff nurses on best practices in wound and ostomy care.
