As a Social Worker, you will prepare our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect from us by ensuring our residents social and emotional needs are met during their stay, coordinating resident discharges and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

• As a minimum, a highschool diploma or equivalent;

• At least three (3) years prior Social Work experience in a Skilled Nursing Facility

• Proficient in the Discharge Planning process;

• Case Management experience
