Position Summary
The Pediatric Cardiac and Pulmonary Transplant Clinical Pharmacist will serve as an integral member of a busy, multidisciplinary heart and lung transplant program at Boston Children's Hospital, providing comprehensive pharmacy services throughout the transplant continuum. Responsibilities will include direct clinical care of pediatric heart and lung transplant patients, medication selection and optimization, therapeutic drug monitoring, drug interaction management, patient and family education, medication reconciliation, and collaboration with physicians, nurses, and other members of the transplant team. The pharmacist will also play a key role in ensuring compliance with CMS and other applicable transplant regulatory and quality requirements, including maintaining appropriate documentation, monitoring program metrics, and supporting ongoing readiness for regulatory review. Schedule is days and every 4th weekend The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
