Job Summary This position provides leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned social work staff. This position is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients.Does this position require Patient Care? Yes Essential Functions -Provide supervision to interns, LCSW's and other Staff in the absence of Manager and/or Supervisor. -Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans. -Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessment when violence is suspected Plan discharge from care facility to home or other care facility. -Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. -Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan. -Knowledgeable in therapeutic behavioral interventions, replacement skill-building strategies, crisis assessment, management, and interventions. -Strong knowledge of HIPPA, Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. -Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner. -Ability to communicate and resolve issues effectively with a broad population of patients, staff and physicians. -Ability to work as part of a multidisciplinary team with a varied population of clients. -Display strong leadership and communication skills to clearly guide peers.
