About the Role Optum's Senior Community Care (SCC) program is seeking a Nurse Practitioner or Physician Assistant to provide care for high-risk Medicare patients in senior housing communities, assisted living facilities, adult family homes, and private residences. This role focuses on comprehensive clinical care, care coordination, chronic disease management, and improving access to healthcare for vulnerable populations.

Key Responsibilities

  • Conduct comprehensive patient assessments, including physical exams, medication review, and diagnostic screenings
  • Support members during short-term sub-acute rehabilitation episodes
  • Document assessments and diagnoses accurately, including ICD-10 coding
  • Collaborate with interdisciplinary care teams to coordinate patient care
  • Conduct advanced care planning and advanced illness conversations
  • Educate and counsel patients and caregivers on treatment plans and disease management

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted state license (or ability to obtain)
  • Current DEA license/prescriptive authority (or ability to obtain, if required by state and role)
  • Valid driver's license and reliable transportation
  • Ability to obtain a collaborative practice agreement, if applicable
  • Ability to work in various patient care settings and environments

Preferred Qualifications

  • 1+ years of APC experience, preferably in long-term care
  • Experience in geriatrics, home care, senior living, or long-term care settings
  • Experience caring for patients with complex medical, behavioral, social, or functional needs
  • Knowledge of chronic disease management, geriatrics, advanced illness, and end-of-life care discussions
  • Proficiency with EMR systems and healthcare technology

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
