About the Role Optum House Calls is seeking a Nurse Practitioner or Physician Assistant to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers to improve health outcomes.

Key Responsibilities

  • Conduct comprehensive in-home health assessments
  • Review medical history and medications
  • Perform physical exams, screenings, and point-of-care testing
  • Identify and document diagnoses for care management and treatment planning
  • Communicate findings to primary care providers
  • Educate members on disease management, medications, and treatment compliance
  • Address social determinants of health and connect members with community resources
  • Recognize and respond appropriately to urgent or emergent situations

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted state license (or ability to obtain)
  • Valid driver's license and reliable transportation
  • Ability to perform assessments in a variety of home environments

Preferred Qualifications

  • 1+ years of clinical experience in family practice, geriatrics, primary care, or home health
  • Experience working with Medicare or geriatric populations
  • EMR and technology proficiency
  • Strong communication and patient education skills
  • Ability to transport equipment up to 30 pounds and navigate stairs

Compensation for this specialty generally ranges from $$109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
