$30,000 Sign-on Bonus or $40,000 Student Loan Repayment Work-Life Balance Flexible scheduling with hours between 8:00 AM and 8:00 PM No on-call responsibilities No weekends required No holidays required About the Role Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers. Key Responsibilities Conduct comprehensive in-home health assessments Review medical history and medications Perform physical exams, screenings, and point-of-care testing Identify and document diagnoses for treatment planning and care management Communicate findings to primary care providers Educate members on disease management and medication compliance Address social determinants of health and connect members with resources Recognize and respond appropriately to urgent or emergent situations Required Qualifications NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care PA: NCCPA certification and active state licensure Active, unrestricted license (or ability to obtain by start date) Valid driver's license and reliable transportation Ability to work independently in various home environments Preferred Qualifications 1+ years of clinical experience in family practice, geriatrics, primary care, or home health Experience working with Medicare or geriatric populations EMR and technology proficiency Strong communication and patient education skills Ability to transport equipment up to 30 pounds and navigate stairs during home visits Highlights Flexible schedule with excellent work-life balance No call, weekends, or holidays Non-prescribing, preventive care model Meaningful one-on-one patient interactions in the home setting Opportunity to improve patient outcomes through education, care gap closure, and coordination with primary care providers. Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
