Community Outreach Worker (Bilingual Spanish/English) Help build bridges to healing, recovery, and community. Central City Concern's Puentes program is seeking a bilingual Community Outreach Worker who is passionate about serving the Latino/a/x community through culturally responsive outreach, engagement, advocacy, and connection to behavioral health services. As a Community Outreach Worker, you will serve as a trusted resource, helping individuals connect to care, navigate services, and achieve their recovery and wellness goals. What You'll Do
- Conduct community outreach and early engagement activities throughout the Portland metro area
- Build trust and meaningful relationships with Latino/a/x individuals and families
- Connect community members to behavioral health, recovery, healthcare, housing, and social service resources
- Provide advocacy, education, support, and resource navigation
- Meet clients where they are, including homes, shelters, clinics, community settings, and places of worship
- Support individuals in overcoming barriers to care and achieving personal goals
- Collaborate with community partners and multidisciplinary teams to coordinate services and improve outcomes
What We're Looking For
- Bilingual Spanish and English required
- Experience working within the Latino/a/x community
- QMHA-R eligible through MHACBO or CADC-R certification required
- Knowledge of behavioral health, recovery, and community resources
- Strong relationship-building, communication, and advocacy skills
- Ability to work independently while collaborating effectively with a team
- Valid driver's license and ability to travel throughout the community

Compensation: $23.86-$31.67/hr. *Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity. External candidates: Offer max is $27.69/hr. per our Union contract. - QMHA-R or QMHA-1
- CADC-R or CADC-2 or 3
MINIMUM QUALIFICATIONS:
- Bilingual in English and Spanish required.
- High school diploma or GED required. Associate's degree or higher in health or social service related field of study preferred.
- Must be registered with MHACBO to meet, at minimum, QMHA-R requirements OR At minimum, CADC-R.
- Must have experience working with the Latino/a/x culture.
- Must have current CPR certification prior to start.
- Must meet CCC privileging requirements as required by FTCA.
- Must have a high tolerance for ambiguity and change.
- Must have a working knowledge of behavioral health terminology.

- Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
- Must be able to climb stairs several times a day.
- Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
- Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
- Must possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial driver's training within 60 days of being an approved driver and continued recertification training.
- Must adhere to agency's non-discrimination policies.
- Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.
RESPONSIBILITIES:
- Provide community outreach and location-based early engagement services to the Latino/a/x community who are not well connected with behavioral health services.
- Provide culturally specific activities that engage, educate, support, and build trust with the community to provide pathways to recovery services.
- Provide opportunity to re-connect to community through culturally specific activities
- Work with Puentes Program Director and Puentes Clinical Supervisor on the quarterly reporting requirements.
- Visit and provide support to clients in their community, home, places of worship, places of recreation, clinics and elsewhere, including outside and in shelters for patients experiencing homelessness.
- Help patients to seek opportunities for further treatment, care, and services within Central City Concern and the community.
- Participate in the development of behavioral health and recovery goals with patients. Support patients to reduce barriers and achieve goals.
- Support patients in advocacy in individual and community needs.
- Bridge community, cultural, educational, and other barriers to encourage self-care and participation in program.
- Work within scope of training and experience and seeks consultation when appropriate.
- Participate in Outreach activities necessary to offer services and engage participation in a non-coercive manner. Escort individuals to meetings and appointments as necessary.
- Collaborate with community service providers to provide necessary case management services and coordinate residential and non-residential treatment and service planning.
- Coordinate services with all members of individual's support network.
- Participate in training opportunities to advance skills and knowledge in addictions and mental health.
- Adhere to all state and federal privacy regulations and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
