Job Summary

Key Responsibilities

  • Provide oversight and direction for weekend case management operations, ensuring effective workflow and timely service delivery
  • Serve as a liaison between department leadership, case managers, and support staff to promote collaboration and problem resolution
  • Support leadership in the development, implementation, and refinement of processes aligned with departmental and organizational goals
  • Monitor and evaluate workflows and outcomes to identify opportunities for continuous improvement
  • Assist in implementing operational and process changes to enhance efficiency and quality of care
  • Partner with leadership to develop and maintain training and educational resources that support staff competency and professional growth
  • Ensure adherence to regulatory, accreditation, and legal requirements related to case management practices
  • Provide guidance and support to weekend staff, addressing escalations and operational challenges in real time
  • Performs interval departmental audits, and oversight of on-going processes that support the department's established performance improvement plan
  • Facilitate mandatory weekend huddles to proactively identify and resolve system-wide barriers
  • Provide direction, assistance, and caseload coverage for the department during staff shortages

Qualifications

  • BSN or MSW required
  • Current Georgia Registered Nursing( RN) license required if BSN.
  • CCM, LMSW, or LCSW preferred
  • Minimum of 3 years of experience in case management, social work, or care coordination required
  • Strong knowledge of regulatory and compliance standards in healthcare
  • Excellent communication, collaboration, and problem-solving skills
  • Ability to lead in a fast-paced, dynamic environment with minimal supervision
  • Ability to effectively manage multiple tasks, activities, and responsibilities
  • Must work every Fri/Sat/Sun or Sat/Sun/Monday
