The Job

We are seeking an experienced and motivated Advanced Practice Clinician (APC) to join our healthcare team based in Riverside, CA. The APC will be responsible for providing high-quality patient care, collaborating with physicians and other healthcare professionals, and ensuring that all clinical practices are in line with the latest medical standards. This specific role requires caring for patients in Skilled Nursing Facilities, in collaboration with our Regional Medical Director.

You Will

  • Patient Care: Conduct comprehensive patient assessments, including medical history, physical exams, and diagnostic tests. Develop and implement treatment plans, including prescribing medications and therapies. Provide patient education and counseling on health maintenance, disease prevention, and treatment options, in particular in Skilled Nursing Facilities.

  • Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care. Participate in interdisciplinary team meetings and case conferences to discuss patient care and treatment plans.

  • Quality Improvement: Monitor and evaluate patient outcomes to identify areas for improvement in care delivery. Participate in quality improvement initiatives and research projects to enhance clinical practices and patient outcomes.

  • Documentation: Maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes. Ensure compliance with all regulatory and documentation standards.

  • Professional Development: Stay current with advancements in medical knowledge and clinical practices through ongoing education and training. Participate in professional organizations and conferences to network and share best practices.

  • Other duties as assigned.

Your Qualifications

  • Nursing Degree; Completion of an accredited Advanced Practice Clinician program.

  • Certification as an Advanced Practice Clinician preferred

  • 3-5 years of related experience preferred

  • Technical expertise - Advanced analytical skills

  • Business Insight - In-Depth understanding of the business

  • Clinical Expertise: Strong knowledge of clinical practices and procedures. Ability to diagnose and treat a wide range of medical conditions, in Skilled Nursing Facilities, in particular.

  • Critical Thinking: Ability to analyze complex patient data and make informed clinical decisions. Strong problem-solving skills to address patient care challenges.

  • Teamwork: Ability to work collaboratively with a multidisciplinary healthcare team. Strong interpersonal skills to build effective relationships with patients and colleagues.

  • Technology Proficiency: Proficiency in electronic health record (EHR) systems and other clinical software. Ability to use technology to enhance patient care and documentation.

  • Time Management: Ability to manage a busy caseload and prioritize tasks effectively. Strong organizational skills to ensure timely and efficient patient care.

  • Attention to Detail: Ability to maintain accurate and detailed patient records. Meticulous in following clinical protocols and procedures.
