Summary:

The Patient Facilitated Services Director is responsible foroptimizingpatient access and coordination on behalf of Philanthropy across thesystem. This role ensures thatphilanthropy contacts have a single point of contact tofacilitatetimely, efficient, and compassionate support throughout their care journey. This role will lead program strategy, design operational workflows, manage schedulinglogistics, and serve as a key liaison between colleagues, community members, providers, administrativeteamsand executive leadership.

This position will be onsite and may include evenings and weekends as needed.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelors degree in healthcare administration, nursing, social work, or a related field. Masters preferred
  2. License/Certifications: Registered nurse, Licensed Clinical Social worker or related field.
  3. Experience: Minimum 6 years of experience in a clinical healthcare setting, working knowledge of healthcare operations, patient services, and care coordination in academic medical centers or large healthcare system.
