Nurse Practitioner or Physician Assistant, Long Term Care - Lane County, OR $30,000 Sign-on Bonus or $40,000 Student Loan Repayment Full-time position; 8-5 pm - Monday - Friday Value Based Care, No Productivity Requirements Work-Life Balance & Benefits No on-call responsibilities No weekends or holidays required 23 days PTO plus 9 paid holidays Optum offers extensive career development opportunities as one of the nation's largest employers of APCs About the Role Optum Senior Community Care (SCC) is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal care to high-risk patients in skilled nursing facilities, assisted living communities, and other senior care settings throughout Lane County, Oregon. Providers work as part of an interdisciplinary team focused on improving outcomes and coordinating care for medically complex patients. Key Responsibilities Conduct comprehensive patient assessments Manage acute illnesses and chronic disease exacerbations Support patients during short-term rehabilitation stays Ensure accurate documentation and ICD-10 coding Collaborate with interdisciplinary care teams Conduct advanced illness and advance care planning discussions Provide education and counseling to patients and caregivers Coordinate care transitions across healthcare settings Required Qualifications NP: MSN or DNP degree with board certification (AANP or ANCC) PA: Graduate of an accredited Physician Assistant program with NCCPA certification Active Oregon license (or ability to obtain) DEA license/prescriptive authority (or ability to obtain) Ability to obtain a collaborative practice agreement if required Valid driver's license and reliable transportation Ability to travel to skilled nursing facilities throughout Lane County Preferred Qualifications 1+ years of clinical experience, preferably in long-term care Experience caring for patients with complex medical, behavioral, or functional needs Knowledge of geriatrics, chronic illness, and end-of-life care discussions EMR proficiency and strong documentation skills Highlights Facility-based Senior Community Care role Focus on high-risk geriatric and post-acute patients No call, weekends, or holidays Strong interdisciplinary team support Opportunity to build long-term patient relationships and improve outcomes for medically complex seniors Travel within Lane County to skilled nursing and senior care facilities. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
