Overview
Territory Patient Admissions Coordinator Pay: $28-$32 per hour, based on experience Benefits: Medical, dental, vision, PTO, paid holidays, 401k, and more! The Territory Patient Admissions Coordinator serves a defined territory and builds strong relationships with clients, caregivers, referral sources, and internal teams to support timely, high-quality admissions. By carefully reviewing referrals against established criteria and gathering complete documentation, the Territory Patient Admissions Coordinator ensures each patient is set up for the best possible care journey. Working closely with agency staff and sales partners, this position helps drive positive outcomes and exceptional satisfaction. With a territory that may span multiple states and evolve, this role offers meaningful impact, collaboration, and the opportunity to support patients at an important moment in their care experience.
