Position Summary

The Physical Therapist is responsible for delivering medically prescribed, goal-oriented therapy in accordance with the Physician's Plan of Care, company policies, and professional standards of practice. This role collaborates closely with an interdisciplinary team to coordinate care and achieve optimal patient outcomes.

Key Responsibilities




  • Develop and implement individualized physical therapy plans of care



  • Provide in-home visits, including evaluation and reassessment of patient needs



  • Communicate effectively with physicians and interdisciplinary care team members



  • Educate patients, families, and caregivers on treatment plans and exercises



  • Participate in interdisciplinary meetings to coordinate and review care



  • Ensure care plans are updated, reviewed, and documented accurately



  • Complete electronic documentation timely and accurately in the EMR (WellSky/Kinnser preferred)



  • Adhere to agency policies, state and federal regulations, and ACHC standards




Requirements




  • Active Florida Physical Therapist (PT) license in good standing



  • Specialized training and experience in Pelvic Floor Therapy



  • Strong knowledge of physical therapy principles and methodologies



  • Excellent written, verbal, and interpersonal communication skills



  • Strong critical thinking, problem-solving, and decision-making abilities



  • Ability to collaborate effectively within an interdisciplinary team




Required Credentials




  • Current CPR certification



  • Level 2 background screening



  • Recent negative TB test or chest X-ray
