Responsibilities Provide skilled nursing care to patients in their homes per the physician's plan of care Perform nursing visits, treatments, and documentation in compliance with Medicare and agency standards Monitor patient condition and report changes to the supervising RN or physician Educate patients and families on care, medications, and treatment plans Maintain accurate and timely clinical documentation Service Area Palm Beach County, Florida Qualifications Active Florida LPN license (required) Home health experience preferred Strong clinical, communication, and documentation skills Ability to work independently in the field Reliable transportation and valid driver's license Ability to pass background screening and drug test Why Work With Activa Home Health?
