Who We Are

The Annex Teen Clinic provides sexual health care and education for young adults through age 25 in a friendly, non-judgmental, and comfortable atmosphere. Our services are low-cost, medically accurate, and developmentally appropriate. The Annex Teen Clinic has been providing services in Northwest Hennepin County and North Minneapolis since 1971.

Wages & Benefits

$22 - $24 per hour. Paid vacation time, sick time, and personal time, plus nine paid holidays per year and two floating holidays; competitive health, dental, and vision insurance offerings; Health Savings Account (HSA) with employer contributions up to $1,200/year for high deductible health plans, and options for Flexible Spending Accounts (FSA); there is employer paid basic life insurance with the option for additional coverage paid by the employee. The employer pays for Accidental Death and Dismemberment (AD&D) and long-term disability insurance. ATC sponsors a retirement plan with a 3% employer contribution after one year of service.

The Health Mentor is based on site at Annex Teen Clinic and provides direct support to young people receiving services. This role delivers one-to-one education in a clinical setting, offers referrals and limited services navigation, and ensures young people can access accurate, developmentally appropriate information during or after their clinic visit. The position also provides classroom-based sexuality education and serves as a resource to Robbinsdale-area schools on sexuality and adolescent health. Responsibilities will include:

Education



  • Facilitating fact-based sexuality education programs for young people on topics including contraception, sexually transmitted infections, healthy relationships, sexual orientation and gender identity, communication skills, and related sexuality topics.
  • Providing one-to-one education in the clinic for young people seeking guidance, support, information, and follow-up related to sexual health, relationships, and accessing care.
  • Providing referrals and limited services navigation, including helping patients understand recommended services, schedule follow-up appointments, and access community resources.
  • Building and maintaining community partnerships that support the Clinic Health Mentor role.
  • Collaborating with partner organizations, schools, and Annex educators to deliver high-quality programming.
  • Collaborating closely with clinic staff to ensure services are confidential, culturally relevant, and accessible.
  • Participating in ongoing professional development related to human sexuality, youth development, and facilitation.


Development and Outreach



  • Conducting outreach to raise awareness of the Clinic Health Mentor role and Annex Teen Clinic services.
  • Creating and implementing a patient referral system in partnership with community partners within the service area.
  • Serving as an expert referral resource for young people, caregivers, educators, and service providers.
  • Participating in professional meetings, strategic coalitions, and workgroups as a representative of The Annex.
  • Recruiting for and/or facilitating out-of-school-time programs as needed.


Reporting



  • Collecting and entering program data in a timely and accurate manner.
  • Completing monthly reports and other required documentation.



Who You Are



  • You have earned a bachelor's degree in education, youth development, human services, or a related field, or equivalent knowledge gained through alternative study and lived experience.
  • You bring a commitment to fact-based, inclusive sexuality education and a belief in young people's ability to make informed decisions when supported with accurate information.
  • You have experience providing fact-based sexuality education and facilitating youth groups or youth leadership programs.
  • You have experience managing work plans, schedules, administrative tasks, and community outreach or event-based engagement.
  • You can manage multiple projects, deadlines, and shifting priorities in a dynamic environment.
  • You are a strong communicator with effective writing, public speaking, and facilitation skills.
  • You are comfortable working in a mission-driven organization serving diverse communities.
  • You are highly motivated, self-directed, organized, collaborative, and skilled at building rapport and trust with youth, caregivers, and community partners.
  • You are innovative and adaptable in delivering services within a clinical setting, including developing new approaches to patient engagement, education, and navigation.
  • You are flexible and willing to work some evening and weekend hours.
  • You possess a valid driver's license, access to reliable transportation, and the ability to meet Annex Teen Clinic's driving record requirements. Please note that if the license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident in which to apply for your Minnesota driver's license or permit.
