Benefit Eligible $7,500 SIGN ON BONUS! Shift: 0.9FTE, 36 hours per week, Day Shift. Job Description: The Social Worker is based at 1-2 Unity Point Health - Meriter primary care clinics and addresses psychosocial and coordination needs for primary care patients of UnityPoint Health - Meriter. This position coordinates care for patients across the lifespan who would benefit from high-touch collaboration, advocacy, and care coordination. Special areas of focus are patients with high risk medical needs (eg multiple chronic conditions, cognitive difficulties, independence concerns, suicidality, etc.), and those with complex social determinants of health needs or barriers (e.g. abuse and neglect, homelessness, financial insecurity, etc.). The Social Worker provides leadership and representation of the ambulatory social work perspective system-wide through providing staff education, acting as a social work content expert, and participating in appropriate system initiatives.
