Overview
Coordinates resources for mental health and addiction services for patients, families, and caregivers upon assessment, during hospitalization, and at discharge. Provides clinical expertise, coordination of services, and acts as an advocate to assist the patient population in navigating throughout internal and external systems in order to facilitate the treatment process and continuum of care. Will serve as a liaison and resource to patients and staff. Helps to eliminate barriers to care and ensure timely delivery of services. As a member of the multidisciplinary team, the Behavioral Health Patient Navigator contributes to quality improvement initiatives regarding pediatric mental health and addiction care. Ability to multitask and float between departments as necessary. Ability to collect, interpret, and analyze data. Excellent computer skills, including but not limited to Microsoft Office Suite. Highly organized with demonstrated skills verbal and written communication, critical thinking skills with the ability to problem-solve and seek assistance from team members when necessary. Ability to work independently. Ability to communicate effectively and work collaboratively with medical and administrative staff. Participates in continuing education activities as appropriate to role. Must be able to tolerate stressful situations with families, patients, and other caregivers.

Compensation and Benefits The compensation range for this position is $22.54per hour - $37.64per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
