Position Description:

The Medical College of Wisconsin's Department of Medicine - Cardiovascular Division is seeking a Nurse Practitioner or Physician Assistant to provide specialty medical care. The APP Hybrid will work as part of a multidisciplinary healthcare team, providing a broad range of diagnostic & therapeutic services- both outpatient & inpatient, and as a member of the Department of Medicine- Cardiovascular Division in Milwaukee, WI.

The Ideal Candidate will be an APP with strong communication skills, strong foundation in evidence-based practice, and ensure an environment of safe quality care, wanting to be part of a thriving and very collaborative academic practice at the Medical College of Wisconsin (MCW).

In this role, you will:

  • Collaborate with interdisciplinary health care teams in diagnosis & treatment of conditions requiring Cardiovascular intervention in a blended environment as a member of the Structural program in both the outpatient & inpatient setting
  • Write accurate and timely progress notes, procedure notes, &/or admission & transfer orders
  • Order, perform, & interpret screening & diagnostic procedures and tests, and appropriately communicate & document information
  • Perform procedures as identified by Outpatient Cardiology services
  • Facilitate & communicate with patients, families, and staff to promote continuity of care across the continuum of care
  • Attend required training, and meet regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission
  • Evaluate quality outcomes for patients through participation in the development of quality indicators & outcomes dashboard
  • Acquire information on the most current development & trends in the PA or NP field through educational programs, inservices, professional associates, & current literature
  • Coordinate & oversee the overall clinic process, ensuring that patient flow from initial referral to post procedure follow up is smooth
  • Develop & facilitate methods to ensure prompt patient care, and coordinate patient flow across multiple inpatient & outpatient sites
  • In conjuction with the medical director, assist with program development & program maintenance in alignment with ACC/AHA guidelines and STS registry requirements to meet changing needs of market evolution & expansion
  • Provide inpatient support for valve patients in ICU & Cardiology
  • Perform a variety of patient visits- new patient referrals, outpatient follow ups, & inpatient consults
  • Assist with creating &/or updating patient education materials
  • Lead the effort to design & implement educational training for physicians & allied health professionals internal & external to F&MCW about the SHD program & the current state of the field of structural & valvular heart disease
  • Develop & implement clinical practice guidelines into the workflow
  • Facilitate patient screening & enrollment in clinical trials
  • Collaborate with the structural heart team to ensure efficient progression of patients through the evaluation process
  • Foster open communication with referring physicians, patients, & their family members

Core Advanced Practice Responsibilities:

  • Perform comprehensive histories & physical exams, identify normal & abnormal findings, including physical, cognitive, & psychosocial status
  • Order, perform, and interpret screening & diagnostic procedures and tests, and appropriately communicate & document information
  • Write accurate & timely progress notes, procedure notes, &/or admission and transfer orders
  • Develop pharmaceutical & and non-pharmaceutical treatment plans
  • Monitor effectiveness of plan of care
  • Educate patients on disease & treatment plans
  • Document within the Electronic Medical Record (EMR), and complete other necessary documents
  • Collaborate &/or consult with physicians and other health care providers as appropriate
  • Educate, counsel, & assist patients and families regarding current health problems, health maintenance activities & preventive behaviors

Preferred Schedule:




Monday - Friday, Hours will range from 7 am - 5 pm, however requires flexibility with start & end times based on business needs



Position Requirements:

Advanced Practice Nurse

  • Wisconsin RN license
  • Wisconsin Advanced Practice Nurse Prescriber License
  • Certification as Advanced Practice Nurse (through ANCC or AANP)
  • BLS and ACLS
  • DEA
  • CITI Certification to be completed within 90 days of hire

Physician Assistant

  • Wisconsin PA license
  • Certification by NCCPA
  • BLS and ACLS
  • DEA
  • CITI Certification to be completed within 90 days of hire


Preferred:
  • At least 4 years of clinical practice (inpatient & outpatient) experience in cardiovascular medicine, OR cardiovascular medicine subspecialty or cardiac surgery. Preference will be given to practitioners with core background in cardiovascular care and ICU care with extensive clinical experience.
  • Acute Care NP certification/licensure and/or experience
