PRIMARY DUTIES AND RESPONSIBILITIES

  1. Assists all patients through the healthcare system by acting as a patient advocate and navigator.
  2. Conducts assessments of patient/family needs by coordinating input from all health professionals and formulating a documented plan assuring continuity of care for the highest risk patients
  3. Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits
  4. Delegates care based on situation while assuming accountability for patient outcome. Supports assistive personnel; serves as a resource and holds assistive personnel accountable to complete delegated tasks.
  5. Along with physician hospitalists/PCPs/specialists, leads and coordinates activities of interdisciplinary treatment team required to make high risk clinical, benefit and network decisions
  6. Identifies appropriate providers, facilities, external healthcare organizations throughout the continuum of care and communicates with an interdisciplinary treatment team to develop and maintain positive working relationships with patients, families and providers
  7. Functions as a coordinator and manager of a defined health population across multiple care settings and for multiple physicians/health care providers or health plan counterparts
  8. Coordinates care across the continuum (inpatient/outpatient/community) to assure appropriate utilization of clinical and community resources
  9. Works collaboratively with provider practices to offer individualized assistance with improving and maintaining quality patient care
  10. Guides development of multiple health care partnerships to achieve positive health outcomes

KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION REQUIRED

  1. Bachelor's degree in Nursing or applicable field
  2. Current unrestricted license as a registered nurse (RN) in the State of Georgia OR an accredited advanced healthcare related degree
  3. Minimum 3 years of clinical care experience
