HealthPartners is currently hiring a Central Intake Specialist with our Hospice department. The Central Intake Specialist supports patients, physicians, other referral sources, home care and hospice staff by performing intake functions to ensure that quality home care or hospice services are appropriately provided, and payer is established to ensure payment for services. ACCOUNTABILITIES:
Serves in primary role of Intake Specialist. Initiates and completes accurate home care/hospice patient intake information in appropriate information system. Screens referrals for appropriateness of home care/hospice service delivery using HealthPartners' policy information and Medicare/Medicaid guidelines and/or provides external customers with alternative resources as needed. Communicate with physicians, nurse practitioners, or physician assistant regarding appropriate orders needed to provide services being requested. Supplies customers with additional data as needed, including prior medical encounters/assessments, verification of demographics, verification of expected/needed home service, benefit coverage, etc. Provides exceptional customer experience. Secures prior authorization for initial visit if required by payer. Provides staff with appropriate information to ensure optimal patient care. Responds to claims, member services, or other internal HealthPartners' staff/areas with questions related to home care/hospice. Performs other duties as assigned for the smooth functioning of the Intake department.
REQUIRED QUALIFICATIONS:
Associate degree in related field, or currently working on College Degree or related experience. Two years of recent customer service experience working in healthcare or related field. Strong critical thinking and problem-solving skills. Proven exceptional customer service skills. Strong oral and written communication skills. Strong computer and typing skills with recent experience with computer systems, such as Microsoft Word, Excel, Outlook, and related applications. Demonstrate strong interdisciplinary and team-building skills.
PREFERRED QUALIFICATIONS:
LPN or CMA with 3 years direct care experience, and 6 months of Skilled Nursing Facility/ Transitional Care Experience, or high paced work environment working with physicians and advanced practice providers/clinicians. Bachelor's degree. One-year recent experience (within the last five years) in home care, hospice, community or senior care with intake, staffing, patient care/planning involvement, or related experience. Knowledge of community resources within the metro area. Knowledge of HealthPartners' computer systems and benefit structures. Knowledge of Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aide, Registered Nurse, and Social Work functions in home care/hospice environment. Knowledge of Medicare/Medicaid home care and hospice benefits.
DECISION-MAKING:
Reviews with manager: Whether to accept marginal case request for assignment of staff. Determining if safe situation for staff or client.
