Assessment Coordinator-PRN Opportunity

West Oaks Hospital has provided psychiatric care to the Houston area and surrounding communities for over four decades. Our 176-bed acute care facility is fully accredited and included a 16-bed residential treatment center for adult chemical dependency. We offer a broad range of behavioral health programs for children, adolescents and adults. Treatment is offered at multiple levels of care including inpatient, partial hospitalization and intensive outpatient.

We are available 24 hours a day, seven days a week to assist you and answer your questions about treatment options and insurance coverage. The hospital is easily accessible off the Southwest Freeway near the Galleria.

West Oaks Hospital is seeking a dynamic and talented Assessment Coordinator to join our team of compassionate, dedicated professionals. The Assessment Coordinator will be responsible for coordination of assessments of incoming patients with Intake Coordinators and Intake Nurses; coordinates doctor's assessments and initiates the admission process. The Assessment Coordinator will also provide technical assistance to the discharge planning process, under the direction of the Director of Intake.

Job Duties/Responsibilities:

  • Conduct intake assessments and screenings for new patients, including psychosocial, psychiatric, medical and risk evaluations.
  • Collect and document patient history, presenting problems and mental status examinations.
  • Determine level of care needed based on assessment findings and clinical guidelines.
  • Collaborate with psychiatrists, nurses and other providers to develop appropriate treatment recommendations.
  • Ensure assessments meet regulatory, accreditation and facility standards (Joint Commission, CMS, state requirements).
  • Provide crisis intervention and de-escalation when necessary.
  • Other job duties as assigned

Qualifications

Requirements:

  • Education Required: Master's Degree in Human Services related field. Preferred: Master's in Social Work.
  • Licensure and certification Required: LPC, LMSW, or LMFT Licensed in Texas; CPR certification; and Handle with Care certification. Preferred: LCSW in Texas.
  • Experience Required: Two years of clinical experience in an inpatient psychiatric setting.
