Salary Range

Annualized: $60,004.41 - $69,478.79

The expected compensation range for this position represents a good faith estimate for this position and the specific compensation offered to a candidate may vary based on factors including, but not limited to, the candidate's relevant knowledge, training, skills, work location, experience, and/or FTE.

Position Summary

The role of the Care Coordinator is to work in partnership with the other members of the multi-discipline treatment team to address the physical health, mental health and overall well-being needs of assigned clients. This work shall be done in a person-centered, trauma-informed manner that may equal sharing responsibilities across positions at times when client needs arise. The primary responsibility of the Care Coordinator will be to ensure coordination of care occurs, referrals are completed, and services are monitored over time.

Location Hybrid; Stillwater, Minnesota

Schedule Daytime during regular business hours, Monday - Friday

Qualifications

Minimum Qualifications:

• Bachelor's degree in social work or related field.
• Two years; case management experience, of which at least one year is experience in community-based mental health with adults and/or children recovering from mental illness or combination of equivalent experience and training.
• This position requires the use of a personal automobile for transportation for business-related travel. Must have the ability to maintain clean driving records, a current driver's license, and auto insurance coverage that meets the requirements set forth by Canvas Health insurance policy limits.

Preferred Qualifications:

• Experience working with and/or delivering services to diverse populations.
• Knowledge of local social and human service networks.
• Experience with Electronic Health Record systems.
