Non-Clinical Patient Navigator - Oncology

Make a difference in the lives of patients and families navigating their cancer journey.
We are seeking a compassionate, organized, and resourceful Non-Clinical Patient Navigator to join our oncology team. In this role, you will serve as a vital link between patients, their families, physicians, and community resources-ensuring no one faces cancer alone.

As part of a multidisciplinary care team, you'll help patients overcome barriers, access services, and feel supported from diagnosis through survivorship. This is a unique opportunity to combine your coordination skills, empathy, and commitment to patient-centered care in a role that truly changes lives.



What You'll Do


  • Serve as the primary point of contact for patients, guiding them through the healthcare system and connecting them to needed services and resources.



  • Coordinate care logistics, including scheduling, access to providers, transportation, medication needs, and support services.



  • Educate patients and families about care plans, available resources, and next steps-referring to clinical staff for medical education as needed.



  • Partner with physicians, nurses, and the interdisciplinary team to ensure patient preferences, needs, and barriers are incorporated into the care plan.



  • Monitor care delivery, attend patient rounds, and facilitate timely scheduling and completion of appointments and tests.



  • Establish and maintain patient pathways from diagnosis through survivorship to promote seamless care.



  • Document patient interactions, track navigation activities, and identify solutions to overcome care barriers.



  • Connect patients and families with internal and community resources, including social work, financial navigation, nutrition, rehabilitation, palliative care, and cancer support programs.



  • Collaborate with organizations such as Hope Connections, Life with Cancer, Smith Center for Healing and the Arts, and the American Cancer Society to expand support opportunities.





What We're Looking For


  • Strong communication and interpersonal skills with the ability to build trust quickly.



  • Experience in patient advocacy, care coordination, or healthcare navigation preferred.



  • Knowledge of oncology care processes and community health resources is a plus.



  • Highly organized, detail-oriented, and comfortable managing multiple priorities.



  • Passion for helping patients and families feel informed, supported, and empowered.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

1. Bachelor's or Social Work degree

2. OPN-CG certification or commitment to achieving in 12 months from hire.

3. Minimum 2 years' experience in cancer field.

4. Familiarity of various local, state, and national cancer awareness, education, outreach, and advocacy programs and organizations.

Salary Range: Minimum 22.41/hour - Maximum 36.98/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
