Title: Medical Caseworker (COVID - 19) Duration: 3-6+ months Contract Pay rate: $30.05/hr on w2 Location: Dallas, TX 75207 Shift timings: Mon - Fri 8 AM - 5 PM BG/DT required: Yes Job Description: Facilitates access to chronic disease prevention and management services with the goal of mitigating the risk of COVID-19 in vulnerable communities. Conduct various chronic disease and risk factors screenings. Preforms tasks aimed at building individual and community capacity by increasing health knowledge through a range of activities including outreach such as home visits, community education events, informal counseling, social support and advocacy. Other duties as assigned.
