~ 32 Hours/week, Full-Time, Day shift ~

Maine state licensure to practice as a Licensed Social Worker (LSW) is required

Summary

The Licensed Social Worker LSW is a member of the Quality Management team and collaborates with members of the interdisciplinary team (patient, family, provider, RN Case Managers, nurses, therapist, etc) in case finding/screening, assessment, planning, implementation, evaluation, monitoring and documentation of the patient's care, psychosocial, and discharge needs.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: LSW Social Worker and/or LSW Social Worker with MSW supervision if indicated by regulations with at least two years of experience, preferably inclusive of experience in an acute care or Clinical Social Worker required.
  2. License/Certifications: Current license with the Maine State Board of Social Work required.
  3. Experience: See "Education".
  4. Additional Skills/Requirements Required: N/A
  5. Additional Skills/Requirements Preferred: Certification or education in Case Management.
