Assures quality documentation and complete reporting in the medical record by accurately assembling all Home Health patient and bereavement charts into specified order and analyzing those charts for any deficiencies. To effectively assure that all records meet Federal, State and Medicaid regulations for certification and licensure and effectively facilitates billing in a timely manner.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. High school graduate or G.E.D. PREFERREDQUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Associate degree in Medical Records. EXPERIENCE: 1. One year previous medical office/records experience, and knowledge of medical terminology CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Maintains good working communications with the medical staff and other customers by politely answering questions regarding the medical record areas 2. Handles telephone communications with the medical staff and other customers in a courteous manner. 3. Answers agency phones when assigned. 4. Rolls phones to and from switchboard when needed 5. Responsible for physician notification letters & IDT updates. 6. Accurately files charting of patient care staff in the medical record. Examines record to assure that all forms pertain to one patient. 7. Analyzes the discharge record for completion of face sheet, certification papers history and physical and appropriate signatures on designated documents per policy and procedures. 8. Assembles discharge folders for patient records as directed by supervisor. 9. Pulls and files records accurately. 10. Assists with data entry into computer system: 11. Keeps records of plans of care that have been sent for signature and logs their return. 12. Responsible for ensuring medical orders are mailed and returned within guidelines set forth in the policy and procedure manuals. 13. Assures that all charts are in the medical records room at the end of the business day. Assures that the medical records room is locked after business hours. 14. Provides receiving unit with a copy of plan of care, physicians orders and med list, is appropriate. 15. Demonstrates knowledge of compliance of Federal, State, and Local regulations. PHYSICAL REQUIREMENTS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the work shift. 2. Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis. 3. Hearing within normal range is required (i.e. To assess breath sounds, bowel sounds, apical pulse, monitors, etc.) 4. Visual acuity must be within normal range. 5. Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Light Work. 6. Must be able to operate motor vehicle with a safe practice. Must be capable of handling information of a confidential nature. 2. Must have the ability to comprehend and perform oral and written instructions and procedures. 4. Must have effective reading and comprehensive skills. 5. Must have strong communication skills, written and verbal. 6. Must have ability to prioritize tasks. 7. Must have independent decision making ability. 8. Must have basic math skills. 9. Must have ability to respond to emergency situations in a calm, professional manner. 10 Must be capable of using computers to input and retrieve information. 11. Must be able to follow written instructions accurately and independently. 12. Must be able to work effectively with a variety of professionals 13. Working knowledge of medical terminology, basic anatomy and terminology. 14. Must possess analytical skills 15. Strong computer skills required. 16. Demonstrates knowledge of compliance of Federal, State, and Local regulations.
