Under direct and indirect supervision, the Bed Coordinator will oversee all bed management activities and patient movement for all Gilchrist Care Centers. The bed coordinator's primary responsibilities are to collaborate with the access and clinical teams to facilitate patient admissions, discharges, intra-Gilchrist Center and inter-organizational transfer processes. Responsibilities include analyzing information received on the Epic Unit Manager and from clinical staff to coordinate the assignment of patients to the appropriate Gilchrist Care Center. The Bed Coordinator will be trained to process Gilchrist service line referrals to assist the access coordinator and care navigation teams as needed.
Education High School, GED, or the equivalent. Experience 3-5 years of administrative experience required. 3-5 years' experience in hospice customer service or experience in healthcare field preferred. Knowledge, Skills and Abilities

- Adept at EPIC and able to make recommendations for build outs to meet needs, run reports, analyze data, and implement process improvement changes to improve quality of services.
- Understanding of the regulatory requirements for hospice levels of care including routine and general inpatient criteria
- Ability to multi-task, set priorities, and manage time effectively.
- Ability to prioritize position duties and responsibilities as well as attend to multiple demands and requests simultaneously and assess situations and act accordingly.

Principal Duties and Responsibilities
- Coordinates assignment of beds based on appropriate level of care and patient aggregation from external hospitals, Direct Admissions, patient transfers within the hospital and transfers from other hospitals identified by the Access Center and/or clinical managers. Includes all entry points into all Gilchrist Care Centers.
- Addresses and resolves daily issues arising from increased census and decreased bed availability.
- Promotes patient, family and physician satisfaction regarding bed assignments. Intervenes in conflicts as needed.
- Escalate and communicate to leadership barriers to transfer to any Gilchrist Care Center.
- Liaises with Nursing and Providers to facilitate patient discharges, inter-facility and intra-Gilchrist Care Center transfer processes.
- Coordinates and manages all Respite transfers including calendar reservations in collaboration with clinical social worker from Gilchrist patient care teams for all Gilchrist Care Centers.
- Promotes optimum utilization of Gilchrist Care Center beds by communicating with Gilchrist Medical Providers, Access Coordinators, Liaisons, Clinical Managers, Associate Clinical Directors of Access, and Directors of Community Programs & Partnerships, and staff on the patient care units regarding bed availability, current census and admission needs.
- Coordinates collaboratively with Access Center on accurate, current/up-to-date census and patient bed availability information.
- Takes the initiative to anticipate problems, uses creative thinking and generates reports as required.
- Coordinates census, bed management updates, and collects and reports monthly data as needed.
- All other duties as assigned.
Licensures, Certifications Licensed Practical Nurse licensure preferred. Physical Requirements
- Ability to concentrate and pay close attention to detail when typing, proofreading and completing assignments.
- Ability to lift up to ten (10) pounds maximum and occasionally lift and/or carry small item
- Ability to stand and walk occasionally to perform the essential functions of the job at random intervals

Pay Range $17.13 - $26.68
Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
