The Senior Living Care Navigator is a non-clinical, non-licensed healthcare resource that conducts telephonic outreach to members, receives requests for assistance from members/responsible parties/facility staff, and partners with Optum advanced practice clinicians (APCs) and nurses (RNs) in supporting member care coordination.

This position is full time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:30am - 5:00pm PST. It may be necessary, given the business need, to work occasional overtime.

We offer 3 to 4 weeks of structured training. The hours of training will be based on your schedule or will be discussed on your first day of employment. First week may be a different work schedule than the normal work hours.

If you reside within the Pacific Time Zone, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities:



  • Telephonic outreach to new members and existing members to introduce or review the Optum care team and plan
  • Support the Optum advanced practice clinician in care coordination activities and administrative tasks


JOB DUTIES TO INCLUDE:



  • Guiding members on utilization of plan benefits such as transportation and the OTC product catalog
  • Completing telephonic assessments
  • Coordinating medical appointments and health services for members under the direction of the advanced practice clinician
  • Facilitating communication between the member and care team for member concerns
  • Providing community resource information to members to address social needs
  • Other duties as assigned


CARE COORDINATION TO INCLUDE:



  • PCP and specialist appointments
  • Diagnostics appointments
  • Dental appointments
  • Vision appointments
  • Hearing appointments
  • DME orders
  • Home health orders
  • Transportation arrangements

Required Qualifications:



  • High School Diploma / GED OR equivalent work experience
  • Must be 18 years of age OR older
  • 2+ years of experience working in a physician's office, SNF, ALF, OR outpatient clinic with patient and medical team communication and collaboration
  • Experience working in cross functional, inter and multi-disciplinary teams
  • Telephonic and written communication skills
  • Experience and proficiency with EMRs
  • Understanding OR experience with medical team collaboration
  • Knowledge of medical terminology

Preferred Qualifications:



  • 3+ years of Clinical OR Medical Record Experience
  • Experience in Microsoft Word (edit, create & save documents), Microsoft Excel (adding, sorting & filtering data) and Microsoft Outlook (email, folders, attachments and calendaring)
  • Background as a Certified Nursing Assistant, Certified Medical Assistant, or LPN
  • Administrative and organizational skills: Ability to self-direct work, multitask, and prioritize multiple deliverables.
  • Experience and proficiency with EMRs
  • Understanding OR experience with medical team collaboration
  • Reside in the state of Washington OR Oregon



Telecommuting Requirements:



  • Reside within the Pacific Time Zone
  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
