Fremont Village, a Lifespark Senior Living Community, is looking for a Lead HHA/CNA to join our growing team!
Fremont Village Senior Living recently opened in October of 2022 and is located in Zimmerman, MN. We help seniors Age Magnificently in our gorgeous 65-unit, apartment style community providing Assisted Living, Independent Living, and Memory Care services. Shifts Available: Full-Time PM (2:30p-10:30p)
HHA Wage: $19-$24
CNA Wage: $19.50-$24.50
Shift Differential: PM: Extra $1/hr

Position Functions and Responsibilities

* Assist clients in getting ready for their daily, evening or night routines as delegated by licensed nurse and according to each client's service plan.

* Assist clients with the following needs according to each client's individualized service plan after being competency tested and evaluated by a registered nurse:

* Assist clients with self-administration of medication and/or medication administration

* Assist clients with body positioning or transferring of a client

* Assist clients with feeding if necessary

* Assist clients with bowel and bladder control, devices, and training programs

* Assist clients with therapeutic or passive range of motion exercises

* Assist clients with bathing

* Assist clients with all activities of daily living as needed

* Transports and/or escorts clients to dining room at meal times

* Respond to client emergency calls and determine what services are needed and contact the appropriate person in accordance with Medical Emergency Policy.

* Clean client apartments as scheduled and according to standard of practice.

* Clean common areas of community as assigned and as appropriate.

* Respond to client concerns and requests, which includes, but is not limited to, checking ill clients, facilitating client interaction, etc.

* Observe and assist clients during meals and report any concerns to the Director of Health Services.

* Document in each client's medical chart the services performed for the client during the shift.

* Communicate pertinent information regarding client status to the next attendant coming on duty before leaving the shift.

* Must be adaptable, flexible to changes, and able to prioritize and organize work efficiently to accomplish workload within time allotted.

* Maintain a positive attitude in relation to customer service program.

* Comply with established safety rules and regulations.

* Promptly reports accidents, incidents, and unsafe and hazardous conditions/equipment to supervisor.

* Understand, comply with, and promote all rules regarding resident's rights.

* Abide by the facility's policies, procedures and practices.

* Participate in and support Quality efforts throughout the facility.

* Attend meetings and mandatory in-services, including orientation and annual training in accordance with company policy and state regulations.

* Knowledge and adherence to safety/disaster preparedness plan.

* All other duties as needed.

Qualifications

Experience and Education

* High school diploma or equivalent is preferred.

* Nursing Assistant Certification is preferred.

* Experience in an Assisted Living community is preferred.

* Trained home health aide who has demonstrated proficiency in performing the technical skills required of their position.

* Minimum of six (6) months experience as a home health aide/CNA.

* Interest in teaching and working with new home health aides and orienting staff to the community.

* Work records demonstrates dependently, responsibility, and reliability.

* Follow Lead Home Health Aide guidelines.
