Responsibilities

Maintains the standards of social work excellence in accordance with the scope of practice and standards of care set forth by the proper state chapters of social work and NASW Standards for Social Work Practice in a Health Care Setting.

Patient Care

Consistently uses a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patient served in assigned service area. Ensures quality and safe delivery of services to patients according to plan of care.

Care Coordination

Conducts assessments of the social and emotional factors related to the patients/clients illness, need for care, response and adjustment to care. Completes appropriate plan of care and interventions according to scope of practice.

Clinical Excellence

Assesses the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources and available community resources. Provides information and appropriate referral action to obtain available resources to assist in resolving patient/family issues.


Qualifications
  • Bachelors' degree in social work
    • Would consider a bachelors' degree in sociology or psychology
  • One year clinical experience in health care
  • Valid Mandatory Reporter course completion by state(s) requirement
  • CPR Certificaiton; maintain a valid Basic Life Support (BLS) Healthcare Provider Card + re-certification
  • Person Centered Care (PCC) course completion within the first 12 months of hire + annual completion of competency validation activities
