Unity Point RN Call Center Clinical Findings Nurse Navigator - Radiology Monday - Friday 8am - 4:30pm, holiday rotation FullTime Benefits 0.8 FTE, 32 hours/week **Position must sit in Compact Nursing State* The Clinical Findings Nurse Navigator, Radiology, facilitates communication and coordinates care with the patient, primary and specialty care physician, and hospital departments on standardized care pathways. Ensures the availability, access, and timeliness of services to optimize quality of care, coordination, and patient experience. Responsibilities
- Ensures all patients that are identified with incidental and/or unexpected findings with recommended follow-up are contacted and offered the option of the recommended follow-up.
- Provides personalized nurse navigation interventions to patients who request additional information, assessment and/or referral. This includes assessment of patient educational level, barriers to learning and other psychosocial factors that may impede remarkable care.
- Directing patients to the correct provider based on the recommendation for follow-up of incidental and/or unexpected findings.
- Partner with the clinics and appropriate provider in assisting patients with appropriate follow-up for incidental and/or unexpected findings at intervals specifically recommended by the radiologist.
- Provide referral and education based on care deemed.
- Utilizes the available software programs to track daily/periodic assignments.
- Maintains frequent communication with local and regional colleagues involved in the Incidental Finding program as well as the Radiology and Oncology departments to facilitate the development of the program and maximize patient care.
Qualifications
Experience:

Requires active and unrestricted license to practice nursing in the states of Iowa and Illinois. -
Requires a minimum of 1-2 years of clinical nursing experience providing direct patient care or equivalent work experience - 2-3 years preferred -
Strong time management and organizational skills -
Possesses excellent written and verbal communications. - Proficiency in use of computer applications such as Microsoft Office and electronic health systems.
- Requires knowledge of federal healthcare laws and regulations.

Requires highly developed communication skills to effectively work with all levels of management throughout the UnityPoint Health, its subsidiaries and affiliates. -
Excellent academic credentials with a track record of professional accomplishments, which demonstrate superior performance, leadership and vision. Ability to work as a team member, creating and maintaining effective working relationships. Ability to understand and apply guidelines, policies and procedures.
Education:
- Graduate from an accredited nursing program. Bachelors of Science (BSN) preferred
- Compliance with Mandatory Child/Adult Abuse Reporting
