Program Overview: The Case Management team provides telehealth and in-person services to clients of all ages in the community who may be on a certification for treatment, transitioning from higher levels of care or have complex care needs. The Case Managers provide wraparound services including, linkage to resources in their community, skill-building, motivational interviewing and safety planning. Peer recovery coaches provide peer mentoring, linkage to community supports, assistance navigating the healthcare system, advocacy on the client's behalf and assisting with communication with providers and other care team members.
