    MaineHealth - Corporate

    The Medical Outreach Case Manager (MOCM) is a professional social services position that is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. In addition to work on-going with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients.

    What you will do:

    • Identify appropriate interventions within program/department parameters, protocols, and procedures. Interventions may include making/tracking referrals to hospital and community resources, agencies and programs, providing health education, and assistance in completion and follow through of in-depth social service applications
    • Contribute to the partnership among care providers, clients, and families to provide safe and effective care; may participate as a member of an interdisciplinary team to support clients

    Responsibilities:

    • Develop and maintain good working relationships with health care providers, community based agencies, and other departments/disciplines
    • Provide timely and appropriate client assessments, care planning, and progress notes as part of care coordination
    • Comply with Employee Health, Human Resources, and HIPAA polices and protocols at all times, and involve MMC Management when responding to complex client situations

    Required Minimum Knowledge, Skills, and Abilities (KSAs)

    1. Education: Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred.
    2. License/Certifications: Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required.
    3. Experience: With a bachelor's degree: Two years of experience in a community health setting, including one year of case management experience required. In lieu of bachelor's degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
