As
a member of the Hospice and Palliative Interdisciplinary Team, the
Palliative/Hospice RN Case Manager implements and coordinates care plans from admission to discharge
for terminally ill patients that provide the care and services to treat
distressing symptoms and improve comfort.
This role educates families and caregivers on recognizing symptoms and
offering emotional and practical support.
What you'll do
- Consult and coordinate with healthcare team members to assess, plan, implement, modify, and evaluate client care plans.
- Direct and coordinate
patient care within established standards of care, hospice guidelines, and company policies and procedures.
- Continuously plan, implement, and evaluate patient plan of care,
incorporating patient, family, and other stakeholders into the process.
- Act as a patient advocate through continuous communication with
physician, patient, patient's family, patient's care team, management, and any contracted services.
- Provide direct care as needed.
- Ensure timely and accurate case documentation within systems of record.
Adhere to Medicare and Medicaid requirements, as applicable.
- Learn and apply Company, Federal, State, and local rules, regulations, and policies.
What you'll need
- Active Registered Nurse (RN) license in Washington State
- Washington State Food Worker Card within 14 days of hire
- CPR and First Aid certification within 30 days of hire
- Ability to understand and communicate in the English language
- Valid Washington State Driver's License and active automobile insurance
