Job Summary

Note: This position is Hybrid, however it will be mostly remote (on-site presence will be on rotating basis)

Duration will be approximately 6 months


The Health Care Navigator supports patients and their families in navigating complex healthcare environments, focusing on enhancing satisfaction, improving care outcomes, and facilitating access to services. The role emphasizes patient-centered communication, problem-solving, and proactive engagement to ensure a seamless healthcare experience.

This position is integral to the Population Health Outreach Team, part of UCSF's Office of Population Health, which oversees strategies to improve care quality and fulfill accountable care contracts.

Key Responsibilities:

  • Patient Support & Outreach:

    • Guide patients through healthcare processes and provide resources to address their specific needs.
    • Foster relationships with high-risk patients and coordinate care during transitions across healthcare settings.
  • Care Coordination

    • Collaborate with multi-disciplinary teams to ensure effective care planning and communication.
    • Schedule appointments, provide reminders, and reinforce post-discharge plans.
  • Quality Improvement

    • Support quality improvement initiatives, tracking outcomes to meet Quality Incentive Program (QIP) metrics.
  • Education & Coaching

    • Provide health education, support patients in setting goals, and reinforce treatment plans using motivational interviewing techniques.
  • Documentation & Workflow Enhancement

    • Maintain accurate records in the medical system and recommend improvements to operational workflows.

Department Description

UCSF's Temporary Employment Program (TEP) recruits and hires temporary employees for immediate clerical and technical support services to UCSF Departments and various off-campus locations. UCSF departments deploy temporary employees to work on special projects, fill in for regular employees who are on vacation or leave, or to temporarily fill a vacant position during recruitment. Frequently temporary employees become successful candidates for career and limited appointment positions.

Required Qualifications


  • BA/BS degree with 2+ years in healthcare or equivalent experience (e.g., EMT, health educator, social work).
  • Electronic Medical Record (EMR) Experience
  • Strong interpersonal and customer service skills.
  • Proficiency with medical terminology and healthcare systems.
  • Ability to manage high call volumes and handle multiple tasks in a fast-paced environment.

Preferred Qualifications


  • Knowledge of EPIC APeX systems and prior experience with quality improvement initiatives.
  • Experience working with older adults or populations with cognitive challenges.
